Many uncomplicated lower urinary tract infections in non-pregnant women aged 16 years and over can be assessed online. The clinician will review urinary symptoms, duration, pregnancy possibility, kidney function, allergies, previous urine results, recent antibiotics, recurrent infections and any symptoms suggesting an upper UTI or another diagnosis.

Fever, rigors, flank or kidney pain, vomiting, severe systemic illness or pregnancy requires a different clinical pathway.

Not every uncomplicated lower UTI requires a urine culture before treatment. A urine sample is more likely to be needed when symptoms are recurrent, treatment has failed, resistance is suspected, the diagnosis is uncertain or there are complicating factors.

A lower UTI usually means infection of the bladder or urethra. Typical symptoms include burning or stinging when passing urine, passing urine more often or more urgently, and sometimes suprapubic discomfort.

Common symptoms include dysuria, urinary frequency, urgency, passing small amounts of urine and lower abdominal discomfort. Cloudy or strong-smelling urine can occur but is not enough on its own to diagnose a UTI.

Visible or microscopic blood can occur with cystitis. However, persistent or recurrent haematuria, especially after the infection has resolved, requires further medical assessment.

Fever, rigors, pain in the back or side below the ribs, nausea, vomiting or feeling systemically very unwell can indicate pyelonephritis. This requires a different antibiotic pathway and should not be managed as uncomplicated lower UTI.

Short-course treatment may sometimes be considered cautiously at eGFR 30 to 44 ml/minute for resistant lower UTI when the expected benefit outweighs the risk.

No. Nitrofurantoin should not be used for suspected pyelonephritis because it does not achieve appropriate tissue concentrations for an upper urinary tract infection.

Repeated infections require a broader review of frequency, culture results, triggers, anatomy, menopause status and possible recurrent-UTI prevention strategies. Repeated empirical antibiotics without review can increase resistance and miss another diagnosis.

Prominent vaginal discharge, irritation or genital symptoms can make a vaginal infection or sexually transmitted infection more likely. The assessment may need to consider an alternative diagnosis rather than treating automatically as cystitis.

If symptoms have not started improving after at least 48 hours, NICE recommends reassessment. Urine culture may be needed and a second-choice antibiotic can be selected according to previous results, resistance risk and clinical suitability.

A switch can be considered when Nitrofurantoin is unsuitable or there has been no improvement after an adequate first-choice trial. The decision should take account of urine culture results, allergy history and clinical progression.

People with significant structural urinary tract disease, recurrent kidney infection, urinary obstruction or other complicating factors may need a different management pathway and should disclose this during assessment.
